DAWC Patient Guide to Accutane (Isotretinoin) 

Accutane (isotretinoin) is a powerful medication used to treat severe acne. It is highly effective but comes with specific guidelines and potential side effects that must be managed carefully. 

Before Starting Accutane

  • Consultation and Consent:

    • Schedule a detailed consultation with your DAWC dermatologist to discuss the benefits and risks of Accutane.

    • Review and sign the informed consent form acknowledging the potential side effects and necessary precautions.

  • Pregnancy Prevention:

    • Women of childbearing potential must use two forms of effective contraception for at least one month before starting Accutane, during treatment, and for one month after stopping the medication.

    • Monthly pregnancy tests are required before, during, and after treatment.

  • Baseline Tests:

    • Blood tests to check liver function, cholesterol, and triglyceride levels.

    • Baseline skin examination and photographs to document acne severity.
       

During Treatment

  • Dosage and Administration:

    • Take Accutane exactly as prescribed by your dermatologist.

    • Usually taken once or twice a day with a meal to enhance absorption.

    • Do not crush, chew, or split the capsules.

  • Monitoring:

    • Regular follow-up appointments with your dermatologist to monitor your progress and side effects.

    • Women will need monthly pregnancy tests.

    • Monthly blood tests to monitor liver function, cholesterol, and triglyceride levels.

  • Skin Care:

    • Use a gentle, non-comedogenic cleanser to wash your face.

    • Apply a moisturizer regularly to combat dryness.

    • Use lip balm frequently to prevent chapped lips.

    • Avoid waxing and other aggressive skin treatments.

  • Sun Protection:

    • Accutane can make your skin more sensitive to sunlight. Use a broad-spectrum sunscreen with SPF 30 or higher daily.

    • Wear protective clothing, hats, and sunglasses when outdoors.

  • Avoid Certain Medications:

    • Avoid vitamin A supplements and tetracycline antibiotics while on Accutane.

    • Inform your dermatologist about all medications and supplements you are taking.

  • Side Effect Management:

    • Common side effects include dry skin, chapped lips, dry eyes, and nosebleeds. Use moisturizers, lip balm, artificial tears, and nasal saline sprays as needed.

    • Report any severe side effects to your dermatologist immediately, including mood changes, severe headache, vision changes, or gastrointestinal issues.
